| Fall Registration Forms |
There are 3 forms that must be completed in order to enroll at Footsteps. Please print the following forms, fill
them out, and either mail them or bring them in to the studio to sign up for classes. Remember to include the
registration fee, any applicable coupons, and payment for your first month's tuition; if you need help
calculating your tuition, just give us a call or send an email with your request to
footstepsfamilydance@yahoo.com. We accept cash, checks, Visa, Mastercard, & Discover, or you can submit
payment electronically via the Paypal button below. Remember, your dancer's place in class is not guaranteed
until all 3 of these forms and payment have been received by our office!

Please include this form in each of your party invitations; have your guests'
parents send the completed form with their child when they come for the
Birthday activities. We must have a signed form for each child who

If you would like to register for any of Footsteps' summer programs via snail-mail, you're welcome to fill
out these forms and send them to us at 8714 Cotter St, Lewis Center, OH, 43035. We encourage you
to call our office at 614-883-5599 for assistance in figuring your tuition--we wouldn't want you to miss
any discounts, and they can be tricky to figure out! Summer registration begins March 1, 2011.
